how important it is to keep
your condition under control.
That’s why we want members
with diabetes to get the tools
and supplies they need.
All Medicare beneficiaries with
diabetes are eligible for up to
two free fasting plasma
glucose-screening tests each
year. In addition, all beneficiaries
are covered for diabetes
glucose monitors, test strips
and lancets.
If you are at risk for complications
from diabetes and your
doctor requests it, you also
are eligible for diabetes self-management
training. Plus,
you can receive a complimentary
glucose meter through
our Diabetes Management
Program. And, you can obtain
glucose test strips and lancets
through a durable medical
equipment (DME) provider.
Depending on how frequently
you have to test your blood
sugar levels, prior authorization
for strips and lancets may
be required.
Syringes and needles are
covered under your pharmacy
program when purchased in
conjunction with insulin.
