you. The free quarterly report offers
a broad picture of your family’s
health care activities and spending,
and may help you plan for Flexible
Spending Accounts or be useful for
tax purposes. Each report includes:
Information on how your health care dollars are distributed among doctors, hospitals and other health care providers
Recommendations on preventive health services for you and your covered family members based on age and gender
Health care information relevant to you and your family
A listing of your health care activities, such as doctor visits, hospital admissions and pharmacy purchases
A summary of out-of-pocket costs, such as copayments and/or deductibles
This report is designed to help keep
you better informed about your
health care dollars and does not
replace your Explanation of Benefits
(EOB). All information is held in the
strictest confidence.
