As a BlueChoice member, you may
receive services from any laboratory
in or out of the network. However,
you will have the lowest out-of-pocket
costs when you receive covered
services from a participating
laboratory.
2. Find a participating lab near you.
There are a number of participating
Preferred Patient Care (PPC) and
Traditional lab providers, including
Quest Diagnostics. Finding a
conveniently located participating
PPC or Traditional lab is easy
using our online provider
directory located at
www.bcbsfl.com, or
call the customer service
number on your member
ID card.
3. Ask your physician to complete the
appropriate laboratory requisition
form. The right form can save time.
It enables the laboratory to correctly
identify the tests your physician
wants performed. It avoids any
confusion caused by handwritten
orders on a prescription pad, which
often require further clarification
from your physician’s office. Finally,
it helps ensure results will be
reported back to the correct
physician in a timely manner.
