You deserve the best service and health care possible. It is important to us that your rights as a member of Dean Health Plan are respected. It’s equally important that your responsibilities as a member are explained.
Your Rights
As a Dean Health Plan member, you have the right to:
- be treated with dignity, respect, and compassion regardless of race, color, age, sex, or creed
- receive a list of Dean Health Plan participating practitioners to help you choose your primary care provider
- present a question, complaint, appeal, or grievance to Dean Health Plan without fear of discrimination
- be given information on procedures and policies about your health care benefits
- be given the name and professional status of any personnel providing health care
- have full consideration of privacy concerning all communication and records pertaining to your care
- get timely responses to requests about your health care plan
- request information about advance directives
- participate with practitioners in decision-making
about your health care
- have a candid discussion of appropriate or medically necessary treatment options for your condition, regardless of cost and benefits coverage
Your Responsibilities
As a Dean Health Plan member, you have the responsibility to:
- read and understand the materials provided by Dean Health Plan concerning your health care benefits
- contact us if you have any questions
- present your ID card in order to identify yourself as a Dean Health Plan member before receiving health care services
- notify Dean Health Plan of any enrollment status changes, such as family size or address
- provide complete information to those providing your health care
- cooperate with and follow the advice that you and your health care practitioner have agreed on. We encourage you to ask questions if you do not fully understand the instructions given.
