Do you ever get confused about how to obtain durable medical equipment (DME), such as a wheelchair,
walker, or crutches? Do you wonder if they will be covered by your Dean Health Plan insurance? We want you to have this information to help you understand the process and any responsibility you may have when you are in need of some type of medical equipment.
Examples of DME are wheelchairs, walkers, braces, prosthetics, colostomy supplies, and diabetic supplies including glucose meters. Home infusion (IV) therapy is also included as a DME benefit.
General Guidelines for DME
- If you are an HMO member, you must receive DME from a plan DME provider. Diabetic supplies can be obtained from a plan pharmacy as well as a plan DME provider. In some cases, a DHP provider may supply you with DME items such as splints or braces.
- When a physician orders DME for you, contact our plan DME providers directly to determine if the item is covered by your policy and if you need prior authorization.
- DME from a non-plan provider always requires prior authorization.
- Your Schedule of Benefits and Member Certificate have valuable information about DME, including any copayments you may be responsible for. Please
refer to it.
- Call Dean Health Plan at 1-800-279-1301 or 1-608-828-1301 if you have any questions.
