We Cover Insulin and Disposable Supplies for People With Diabetes

Supplies for the treatment of diabetes purchased from a provider of health care for the treatment of diabetes are a covered benefit on your policy. These items must be purchased at a plan pharmacy if you are an HMO member. If you are a Point of Service or Triple Option member, you may buy these items at non-plan pharmacies, but higher copayments or deductibles may apply.

Disposable supplies include blood or urine glucose strips, control solutions for blood glucose monitors, alcohol swabs, cotton swabs, finger stick devices, lancets, and syringes. Insulin and disposable diabetic supplies are subject to copayments or prior authorization guidelines as specified in your Schedule of Benefits and are dispensed in a 30-day quantity unless otherwise specified in your Schedule of Benefits. Single packaged items such as blood glucose sticks are limited to two items per copayment.

Insulin Pumps and Blood Glucose Monitors
Insulin infusion pumps and blood glucose monitors are also covered and are subject to the durable medical equipment guidelines and copayment specified in your Schedule of Benefits. Insulin infusion pumps are lim-ited to one per calendar year. You must use the pump for 30 days before purchasing.

Please remember that insulin infusion pumps, blood glucose monitors, and diabetic self-management educational programs must be prior authorized by Dean Health Plan. You can refer to your Schedule of Benefits regarding your specific coverage and prior authorization guidelines.

Use the Mail-Order Service
Dean Health Plan and Dean Pharmacies offer a mail-order service for you to obtain your test strips and lancets. You can take advantage of this even if there is not a Dean Pharmacy near your home. Check with your local pharmacy where you currently get these supplies and ask if they can provide this same service to you.

Here are the steps you need to follow to take advantage of the Dean Pharmacy mail order service. The first time you use the mail-order service, mail the prescription order for the test strips and lancets to:

You can get a prescription order from your physician.

Include your name, date of birth, address, telephone number, Dean Health Plan member number, and group number (you can find these on the back of your Dean Health Plan ID card). Indicate that you would like to have your test strips and lancets mailed to you.
