The law requires your doctor to protect your medical record and make it available to you. You can get a copy of your medical record from your doctor’s office. If there is information in your medical record that you would like to comment on, you can add written comments. If your doctor asks you questions that you feel uncomfortable answering because they will be recorded, let your doctor know. Be clear about the information you don’t want included. (In some instances, physicians may not be able to comply with your request to exclude information about your condition due to its relationship to your overall health.) Your medical record includes:
- doctor’s notes of each visit
- lab test results
- hospital visits
- medicines, including any allergies to medicines
- your medical history
- details about your lifestyle
- family medical history
If you see a specialist, you may need to fill out a release form so that your doctor can release your medical information to him or her. You have the authority to limit the access a requester, such as a specialist, has to your record. For example, if you have a broken arm, you can write instructions on the release form to release only the information about your broken arm.
