Are
you getting a bill from an ambulance company for
recent services?
You may want to check your Dean
Health Plan (DHP) policy. Your policy may cover
ambulance services.
What should you do when you
receive the bill?
Many ambulance companies do not bill insurance
companies. If you receive a bill, you can send
it to Dean Health Plan yourself for processing.
(You may want to keep a copy of the bill for your
records.) If you have any questions regarding
your bill, please contact our Customer Service
Department.
Where do I send the bill?
If your bill was sent to Medicare first, please
include a copy of your Explanation of Medicare
Benefits when you send your bill to us. You can
send the bill to:

Please remember that it is important
to check your Schedule of Benefits of your Dean
Health Plan policy to see if you have coverage
for ambulance services. If you are not sure what
your policy covers, please contact our Customer
