As a smart health care consumer, you want to take advantage of all the benefits your health plan has to offer. One of the value-added benefits Health Options offers is the BluePrint for Health program — a program offered at no additional cost to our members.

BluePrint for Health is designed to help members with chronic or long-term conditions take an active role in managing their own health. Through the program, members learn about their condition and ways to control it in order to improve their quality of life.

Registered nurses with extensive experience and training work with the members, their families and their physicians, reinforcing doctors’ treatment plans and teaching members coping skills.

Diabetes
Members learn the importance of managing their diabetes, preventing complications and following their doctor’s advice. Emphasis is placed on American Diabetes Association guidelines. Nurses stress the importance of routine medical checkups, periodic blood testing for glucose control, blood pressure monitoring, foot care, diet and exercise, annual eye exams and other tests.

Congestive Heart Failure
The goal of the program is to help members improve their understanding of heart failure and help them manage their disease. Members are encouraged to monitor their diet and weight, take their medications and follow their doctor’s treatment plans.

Healthy Addition
This program is designed to encourage good preventive care for expectant mothers. Nurses provide information on healthy habits, prenatal information and emotional support.

Other programs
Members in North Florida can enroll in our asthma program to learn about asthma and how to use a peak-flow meter to anticipate asthma attacks and avoid them by adjusting medications. Among the results: fewer trips to hospital emergency rooms.

Members in South Florida have access to cancer and orthopaedic programs.
