BluePrint for Health is designed to help members with chronic or long-term conditions take an active role in managing their own health. Through the program, members learn about their condition and ways to control it in order to improve their quality of life.
Registered nurses with extensive experience and training work with the members, their families and their physicians, reinforcing doctors’ treatment plans and teaching members coping skills.

Members learn the importance of managing their diabetes, preventing complications and following their doctor’s advice. Emphasis is placed on American Diabetes Association guidelines. Nurses stress the importance of routine medical checkups, periodic blood testing for glucose control, blood pressure monitoring, foot care, diet and exercise, annual eye exams and other tests.

The goal of the program is to help members improve their understanding of heart failure and help them manage their disease.
Members are encouraged to monitor their diet and weight, take their medications and follow their doctor’s treatment plans.


Members learn about asthma and how to use a peak-flow meter to anticipate asthma attacks and avoid them by adjusting medications. Among the results: fewer trips to hospital emergency rooms.

This program is for members who most need help. They may be suffering from multiple diseases or be facing financial, transportation, family, or psychological and social issues. The goal is to maximize their ability to function as independently as possible. Nurse case managers coordinate the member’s enrollment in one of the BluePrint for Health programs; an in-home social worker visit to determine needs, such as a walker; and referrals to financial advisors or locally active health organizations and support groups.


Members get help with the complexities of cancer treatment.

Nurses coordinate
inpatient and outpatient care, as well as diagnostic tests and physical or occupational
therapy, for members who need such orthopaedic services as joint replacements.
