  4. If you are admitted to a participating hospital, you always will be treated by a participating physician.
    True   False

Answers

  1. False. Coinsurance is a percentage of the allowable costs you must pay. A copayment is a fixed dollar amount that you may be required to pay for office visits.
  2. True. Your children are covered from birth to age 16 for the full range of immunizations, including diphtheria, tetanus and pertussis; chicken pox; hepatitis A and B; haemophilus influenzae; and measles, mumps and rubella (German measles).
  3. True. Your plan allows you to see physicians outside of your network of preferred providers, but you will incur higher out-of-pocket costs, for which you are responsible. Remember, too, that Blue Cross and Blue Shield of Florida regularly evaluates physician performance and sometimes makes changes in its network of independent contracting health care providers. To avoid unnecessary out-of-pocket expenses, it’s a good idea to verify a physician’s participation before making an appointment.
  4. False. Sometimes members go to a participating hospital and receive services from nonparticipating physicians, such as anesthesiologists or radiologists. They are entitled to charge patients their customary fee, even if the amount exceeds Blue Cross and Blue Shield of Florida’s allowable limit. You may be responsible for paying these physicians the difference between their charge and the amount allowed by your plan, called balance billing. To help reduce your out-of-pocket costs, ask for physicians who participate with Blue Cross and Blue Shield of Florida. In most cases, your request can be accommodated.
