Chronic Disease Management
program can help. We can
provide educational material
about your condition and help
support your physician’s
treatment plan.
The program focuses on
promoting awareness and
compliance with the American
Diabetes Association’s Standards
of Care. Nurse case
managers will help you
identify any warning signs
of complications. Stress is
placed on the importance of:
- Routine medical checkups
- Following your doctor’s treatment plan
- Annual eye exams and other tests
- Blood sugar testing
- Blood pressure monitoring
- Foot care
- Diet and exercise

Members who have diabetes
can receive a complimentary
glucose meter through our
Diabetes Chronic Disease
Management program. You
also can obtain glucose test
strips and lancets covered by
your health plan through a
